1. Print and sign the "Autorization to Obtain Emergency Medical Care" form

2. Print and sign the "Release, Waiver of Liability and Assumption of Risk" form

3. Once signed, get these two forms to your coaches

1. Print and sign the roster form below

2. Once you've filled out that form below (print, fill out your roster and sign), then also SUBMIT YOUR ROSTER HERE

3. Bring each of your player's "Autorization to Obtain Emergency Medical Care" forms and "Release, Waiver of Liability and Assumption of Risk" forms, as well as your signed roster form to registration in College Station.

All players are required to wear a mouthpiece and a cap-style soft shell helmet while participating (see rules for definition of soft shell helmet)

Entry Fee

The cost is $500 per team with a maximum roster size of 20 players.  

Bring entry fee with you to registration.  Sorry, we are not set up for online or advance payments.
